The increase incidence of chronic disease presents a considerable challenge to the NHS. This module addresses the issues of pertinent healthcare in the management of a range of chronic diseases and therefore will enable and foster a culture of caring for those patients requiring ongoing support. This course is relevant to a mixture of healthcare professionals working in either primary, secondary or pre hospital environments and is delivered via a blended learning model of classroom and online learning.
